At Augustana College, your safety and well-being are our top priorities. Whether you need to report an incident, provide a witness account, or explore your rights and support options, we are here to guide you.

Use our simple, online reporting system to submit concerns regarding student conduct, Title IX, bias, discrimination, harassment, and general campus safety.

How to file a report

Access our online reporting portal and choose the category that best matches your situation. Available forms include:

Bias, Discrimination, and Harassment

Use this form to report actions, behaviors, or communications that intimidate, demean, mock, degrade, stereotype, marginalize, or threaten individuals or groups based on actual or perceived identities, including age, ancestry, ethnicity, national origin, ability, sex, gender identity/expression, citizenship status, marital status, socio-economic status, race, religion, sexual orientation, or veteran status.

Title IX / Sexual Misconduct

Use this form to report discrimination or harassment based on sex, sexual orientation, or gender identity, including sexual assault, dating violence, stalking, and conduct that creates a hostile environment.

Code of Social Conduct Violations 

Use this form to report a violation of the College’s Code of Social Conduct on campus.

Housing Code of Conduct Violations  

Use this form to report a violation of the College’s Housing Code of Conduct.

Hazing Incidents 

Use this form to report any activity or conduct that may constitute hazing.  

Additional forms: You can also submit an Honor Code Violation or share feedback via the President’s Suggestion Box through the reporting portal.

Unsure which form to use?

If you are uncertain which category applies to your situation, please submit your report using the General Incident Report. The triage team will review the report and ensure it is referred to the appropriate office.
