Flu shot clinic

The Rock Island County Health Department will offer a flu shot clinic for students from 9 a.m.-2 p.m. 

No appointment is necessary. Enter through right side parking lot door of Centennial Hall to ensure physical distancing; the lobby will be exit-only.

Before you arrive
 
You must print and fill out the medical consent form, and bring a photocopy of your insurance card (both sides) and a photo ID. Paper forms will not be available the day of the clinic.

If you have any of these insurance providers, your shot is covered: 

Blue Cross Blue Shield of Illinois PPO
Health Alliance
Gallager
Humana (Medicare)
Illinicare (Medicaid)
Medicare Part B
Meridan (Medicaid)
United Health Care

Call the number on your insurance card to make sure the Rock Island County Health Department is in-network and the flu vaccine is covered, especially if you have out-of-state insurance.

If you don't have insurance, or the vaccine is not covered on your policy, the cost is $35. Bring cash, check, or debit/credit card.
