DESCRIPTION:The Rock Island County Health Department will be offering flu shots for Augustana employees and students on Wednesday\, Nov. 5\, 2025\, from 9 a.m. to noon in Centennial Hall. Please enter through the back door of the building and exit through the front. This is a walk-in clinic and no appointment is necessary.Individuals must bring the following items to the clinic:A copy of your insurance card (front and back)\,Completed consent form. Blank consent forms will be available at the clinic as well.This consent form document includes the consent form as well as a list of accepted insurance programs. It is recommended that you call the phone number on your insurance card to make sure the Rock Island County Health Department is in-network and that the flu vaccine is covered\, especially if you have out-of-state insurance. For those enrolled on Augustana medical insurance through UMR this is considered an in-network provider and insurance will be accepted as coverage.If you do not have insurance\, or your insurance is not accepted\, the cost is $66 for the shot payable by cash or check payable to Rock Island County Health Department.Please note: COVID shots are NOT available at this clinic.
