Sports medicine presentation

The sports medicine presentation is mandatory for all student-athletes participating in NCAA intercollegiate sports at Augustana. Enrolling in the course requires an Augustana email and password. Follow the directions carefully. After viewing the presentation, students must take the Moodle quiz and get a 100 percent, otherwise the viewing will not count.

Student-athletes will not be allowed to participate in intercollegiate athletics until they have viewed the presentation and taken the quiz. Please contact Denise Yoder with any questions.

Each student athlete is required to have a physical examination prior to participation in any intercollegiate sport. Please click on the appropriate link and download the forms. Fill them out and send them back as soon as possible to Augustana College, Sports Medicine, 639 38th St, Rock Island, IL 61201. Forms are due Aug. 1.

The final decision on athletic participation status will remain the responsibility of Augustana's team physician upon thorough review of the athlete's medical information. A student athlete who doesn't return a complete pre-participation physical form prior to the onset of athletic participation will be responsible for scheduling and payment of a physical examination either from our team physician or a medical physician of your choice.

Students will not be allowed to participate without a physical, the following completed forms and proof or waiver of sickle cell trait testing.

Intercollegiate Athletic Insurance

All intercollegiate athletes and cheerleaders who have been properly enrolled as such, are covered under a master intercollegiate sports insurance plan held by Augustana College.

The Intercollegiate Athletic Insurance Plan carries a $2,500 deductible. The Student Accident and Sickness Insurance offered to all students is designed to cover this deductible. Enrollment is automatic unless a signed waiver is submitted online to Parents and students should exercise this option to participate in the plan based upon the procedures and benefits available from their family coverage. If a signed waiver is submitted online to by Sept. 3 each year, the fee for the Student Accident and Sickness Insurance Plan will be deleted from the fall term bill.

It is the responsibility of the athlete to initiate the insurance procedure and to assure the claim and bills have been submitted for benefit payment. Gallagher Koster is responsible for processing your claims.
