Accident Investigation

When an Augustana Employee is involved in an accident on campus grounds, the following Accident Investigation Forms should be filled out completely:

  • The Supervisor's Report of Injury Form is to be completed by the Supervisor of the injured Augustana employee as soon as possible after the accident occurs.

  • The Employee Statement Form is to be completed by the injured Augustana employee as soon as possible after the accident occurs.

  • The Accident Investigation Form is to be completed by the Supervisor or General Manager of the injured Augustana employee within 72 hours of the occurrence of the accident.

All of the Accident Investigation Forms are to be sent to the EHS Office or scanned and emailed to the Safety Office.
