Participation in Varsity Sports
Each student athlete is required to have a physical examination prior to participation in any intercollegiate sport. Please click on the appropriate link and download the forms. Fill them out and send them back as soon as possible to Augustana College, Sports Medicine, 639 38th St, Rock Island, IL 61201. Forms are due Aug. 1.

The final decision on athletic participation status will remain the responsibility of Augustana's team physician upon thorough review of the athlete's medical information. A student athlete who doesn't return a complete pre-participation physical form prior to the onset of athletic participation will be responsible for scheduling and payment of a physical examination either from our team physician or a medical physician of your choice.
Students will not be allowed to participate without a physical, the following completed forms and proof or waiver of sickle cell trait testing.
