Before you begin, it is important that you gather the information you need for your students. For security and privacy reasons, you cannot finish this form from a different computer once you have began. Here is a list of information you need from the students you are nominating:

Chair, and # of years in chair
Grade
Other honor bands they attended
Whether they take private lessons, and who their private lesson teacher is
